The availability of this program is managed through the Homeless Coalition of Polk County. In order to qualify, the individual/ family should be currently experiencing homelessness; at risk of homelessness; fleeing or attempting to flee domestic violence, dating violence, stalking, sexual assault; or include a veteran family that meets one of the preceding criteria. In order to apply, they must contact the Homeless Coalition of Polk County at 863-687-8386. Lastly, please note that the family will not be assisted by LHA if they are not referred by the Homeless Coalition.
